In order to ensure that client needs are met and that Five Acres’ programs are effective as well as meet or surpass industry standards, Five Acres' research department works continuously to assess and compile program and client data. The following 2007 statistics reflect the research department’s findings.



Adoption Promotion and Support Service

57% of clients participated to prevent disruption of their adoptive families.
43% participated in APPS to facilitate adoption process.
92% of clients
remained active, were adopted or returned to their biological families

Aftercare

39 children served
Services delivered to children for and average of 6.4 months following residential placement.
84%
were successful over the last two years maintaining placement stability for three to six months following residential placement.

Deaf Services
78 families served
87% of mothers in the perinatal program who gave birth in 2007 had healthy pregnancy outcomes.
92% of families were actively involved in their children's school-based mental programs.
An average of 93% of infants/toddlers served met normal development milestones and received timely immunizations.

Education: Therapeutic Non-public School
110 students served
29% of the student body was comprised of day students from the community.
89% students met their goals in reading.
15 students were mainstreamed into public school.
Students improved by 1.0 grade level per year, a 67% increase from their previous school history.

Family Preservation Network

266 families served
More than half of new clients served in Pasadena office were "alternative response" families designed to provide short-term early intervention and prevention services.
90% of families remained intact at case closing over the past four years.


Foster Care

130 children served
More than 70% of foster families have been certified for at least two years and nearly one-third have four or more year's experience.
More than 90% of children had a permanent person involved in their life.
56% of children exiting the program achieved permanence through reunification, adoption or legal guardianship and an additional 23% continued family care.

Grace Center
162 families served
Nearly 120 information and referral calls were received each month.
95% of clients had a safety plan and 100% of clients had a safe place to stay after participating in the program.

Group Homes
24 youth served.
85% received some type of special education.
100% of youth were connected to a permanent person in their lives for second year in a row.
 
81% had family involvement and all youth with our families received family-finding services.

Home-/School-based Services
 
378 families served
91% of families were actively and positively involved in their children's counseling foe a second year in a row.
85% success rate in treatment goals was achieved in Pasadena and El Monte community sites.


Independent Living Services
22 youth served
70% successfully held part-time or full-time jobs
68% were enrolled in higher education or vocational programs


Multidisciplinary Assessment Teams
8 families served
100% were either returned to their parents or remained safely in care of relatives or foster families,

The Residential Treatment Center
131 children served
10% more children served over the last three years, a period when fewer placements for children were available nationwide.
The children served had increasingly complex needs including more extensive histories of psychiatric hospitalization.
90% of children had a permanent person involved.
Children stayed in the program for shorter periods and when leaving the program, 96% children moved to less restrictive settings - 47% went to live with parents, relatives or foster families.

Therapeutic Behavioral Services
105 children and youth served
Focus of services was to prevent placement in higher levels of care (73%) or to enable transition o family-based care (27%).

92% were achieved their placement goals.

Wraparound
37 families served
Clients were referred by LA Co. DCFS and Department of Probation to maintain youth placement in family settings (70%) or support transition from group care to families (30%).
80% of clients served successfully maintained placement stability or transitioned back to families from group care.
