Outcomes

 
 

To ensure that Five Acres’ programs are effective and that the more than 4.500 children and their family members needs are met while meeting or surpass industry standards, Five Acres' research department works continuously to assess programs and compile program and client data. The following 2009 statistics reflect the research department’s findings.
 

COMMUNITY-BASED PROGRAMS
Home-/School-based Services 1053 family members in 283 families served
82% of families were active in their children’s treatment plans.   
Outcomes exceeded Department of Mental Health averages with 84% of families reporting improved family relationships compared to a figure of 71% countywide.


Deaf Services
  (Est.1999) and Perinatal Services (est. 1995)
221 children and family members.
90% of perinatal program deaf mothers had healthy pregnancies.

Family Preservation Network  (Est. 1995)
263 families and 999 children served
92% of families remained intact at the end of service.
Families’ levels of participation (89%) and progress (92%) were excellent.

PERMANENCY
Group Homes  (Est. 1972)
18 youth served representing 123 family members
83% Had a “permanent person”
100% of youth with family connections were involved in treatment planning by year-end.


Foster Care  (Est. 1988)
110 children served
16 new resource families were certified during the year.
93% of children had a “permanent persons” involved in their lives.
DCFS performance scorecard showed the Five Acres met or exceeded every performance standard for child safety, permanence, and well-being.


Adoption Promotion and Support Services  (Est. 2005)
323 children and their family members served
The primary reasons for referral were to support adoptions in progress and to work with hesitant children and parents.
85% of children were pre-teenagers, age 12 and under.
80% of children in foster-adoptive families and adoptive adoptive families actively participate in services.
Over the last five years, Five Acres has placed 32 children in adoptive families.

78% of youth successfully held part-time or full-time employment
72% were enrolled full-time in higher education or vocational programs
